Please fill out the appropriate form(s) at home and bring them completed to your initial visit. Your chiropractor and/or massage therapist will discuss the content of each before your first treatment.
Directions:
From the 401: Exit Neilson Road (East of Markham Road and West of Morningside Ave) and drive South. Cross over Ellesmere where Neilson Road becomes the hospital roadway. Park in the Main lot on the right.
By city streets: Drive along Ellesmere Road to Neilson Road turn south into the hospital. Park in the Main lot on the right (Southwest corner).
From Downtown Toronto: Take the DVP North to the 401 East. Travel East to Neilson Road (1st exit past Markham Rd.). Turn South (right) on Neilson, this road takes you over Ellesmere onto hospital road, park on right at main parking lot (Southwest corner).

Our office is easy to find:
Go into the Medical Suites area, called “the Court”, through the main doors of the hospital at the circular drive. Turn left immediately and use the 1st set of stairs or elevator to go up one flight to the 3rd floor. CCHC is located in suite 310.
